JOB OVERVIEW

Modality Partnership Nottingham Division has a part-time vacancy (20 hours per week) seeking a skilled and compassionate, experienced & qualified General Practice Nurse to join our healthcare team. As a Practice Nurse, you will play a vital role in providing high-quality patient care and assisting in various medical procedures working at Wellspring Surgery. If you are passionate about helping others and have a strong knowledge of anatomy, we encourage you to apply. PRINCIPAL ACTIVITIES – EDUCATION AND DEVELOPMENT

  • To keep abreast of advances in clinical practice and identify personal training and development needs.
  • Support colleagues in their development by encouragement, informal teaching and coaching.
  • To attend meetings as directed by the Practice Manager and Partners.
  • To attend mandatory training identified through the appraisal system.

KNOWLEDGE:

  • Knowledge of management of patients with chronic conditions.
  • Knowledge of accountability of own role and other roles in primary care.
  • Knowledge of health promotion strategies.
  • Knowledge of local and national health policy.
  • Wider health economy awareness.
  • Knowledge of clinical governance issues in primary care.
  • Knowledge of patient group directions and associated policy.
  • Knowledge of public health issues.
  • Ability to identify determinants on health in the local area.
  • Knowledge of public health issues in the area.

SKILLS:

  • Clinical leadership skills.
  • Clinical skills cervical cytology, immunisation and vaccination, ear care, minor surgery.

Responsibilities

MAIN DUTIES OF THE JOB

The role is both the delivery of evidence-based practice for patients presenting with a chronic condition, and the provision of preventative health care to the practice population. for the care delivered to their patients, demonstrating critical thinking and skills in clinical decision-making in the management of patients.
The role is not a regular 9am-5pm job; the job requires innovation, flexibility, and commitment and the postholder will be required to work resourcefully as part of the team to ensure tasks are completed.

KEY DUTIES & RESPONSIBILITIES:

  • To promote and maintain high standards of nursing care in accordance with the NMC Code of Professional Conduct and Guidelines for Professional Practice.
  • To provide travel immunisations, cervical cytology, ear syringing, wound management.
  • To plan and run health promotion and long-term condition related clinics. Ensure that the care given meets the requirements of local and national guidelines, policies and standards including the Quality Outcome Framework and working the rules and guidelines laid down by the NHSE/ICB and practice.
  • To directly communicate with patients on matters relating to their ongoing and highly complex health needs giving due consideration to the likelihood that there will be emotive issues which will require developed skills of tact, diplomacy, and empathy.
  • To ensure that patients have detailed assessments made of their healthcare needs and that individual programmes of care are planned, implemented, and regularly reviewed.
  • Facilitate patient referrals to appropriate clinical services.
  • To liaise with other disciplines and outside agencies with a view to providing holistic healthcare, whilst maintaining patient confidentiality in accordance with the NMC Code of professional Conduct.
  • To participate in the delivery and evaluation of healthcare services at the Practice through meetings, working groups and regular liaison with colleagues.
  • To be responsible for the ordering, safe systems, and processes, storing and administering of all medicines/drugs according to NMC standards.
  • To undertake and participate in clinical audit projects as necessary to own work.
  • Respond to medical emergencies.
  • Ensure that the principles of Infection Control are always adhered to.
  • To have a good awareness of issues concerning child protection & vulnerable adults.
