REQUIREMENTS

• GPhC registered Clinical Pharmacist
• Independent Prescriber (IP) qualification essential
• Minimum 1 year’s experience in a general practice setting
• Strong understanding of LTC management, SMRs, and QOF

ABOUT THE ROLE

We are seeking an experienced Prescribing Clinical Pharmacist to join a thriving Primary Care Network (PCN) covering Peckham, Dulwich and Camberwell.
This is an ARRS-funded role with a strong clinical support network, ideal for a pharmacist looking to deliver high-quality care while being part of a collaborative team.

ROLE SCOPE

• Medicines Management – optimise prescribing, support safe and cost-effective medicines use.
• Long-Term Conditions (LTC) Management – manage patients with conditions such as hypertension, asthma, diabetes, and COPD.
• Structured Medication Reviews (SMRs) – identify and address polypharmacy risks.
• Quality & Outcomes Framework (QOF) – contribute to meeting clinical indicators.
• Provide supervision to junior team member
