About the Role  Under the supervision of the Supervisor, the Case Planner will be responsible for case intake, management of cases and providing service needs, in accordance with ACS standards and indicators and Rising Ground policy and procedure manual.   What You’ll Do * Responsible for timely submission of all paperwork, completion of intakes as well as progress notes and all necessary paperwork  * Coordinate all case management, referral and advocacy services for each family. * Develop resources for social services referrals, assessing, coordinating and evaluating preventive services needed by the family * Provide individual, family and group counseling. * Maintain case records in an orderly fashion. * Home visits must take place weekly and may require case planner to walk up steps. Evening hours may be warranted for some cases.  Your Qualifications * Bachelor’s degree in Social Work or in related field preferred.  * Good writing and oral communication skills; computer literate. * 2-3 years experience in child welfare field strongly preferred. * Excellent time management skills with the ability to meet deadlines in a timely manner while managing competing priorities. * Bilingual, (preferably Spanish, French/Creole) to conduct case management for our families. * Excellent written and oral communication skills. * Proficient use of Microsoft applications; knowledge of PROMIS/CONNX desired. * Cultural awareness and sensitivity when serving clients that come from different backgrounds and walks of life. Supervisory Responsibility No Work Environment: Clinic/Field/Corporate Office Position Type/Expected Hours of Work:  Full time position with office location 91-14 Merrick Blvd Queens 11432, scheduled appropriate to the needs of client and families. This position calls for a thirty-five (35) hour work week, but more than 35 hours per week may from time to time be required. Additional Requirements: * Authorized to work in the U.S. * Ability to work in-person in New York City (NY) for at least 3 days or more per week. * Ability to travel to other Rising Ground sites (if required) * Ability to travel to various locations, including weekly home visits. Work-related travel expenses will be reimbursed by agency.
