We are seeking a Registered Nurse who is passionate about promoting health and wellness equity in the Downtown Eastside (DTES) to join the Primary Care team at Kilala Lelum Health and Wellness Cooperative as Co-Lead. KL’s aim is to provide healthcare services through an outreach-based, culturally-safe, trauma-informed, member-centered, and harm reduction approach. The PC Co-Lead (RN) will support primary care staff by overseeing care coordination and programs, managing internal and external referrals, providing clinical oversight, training and mentorship for nursing staff..
This role combines strategic leadership (40%) with hands-on clinical support (60%), ensuring culturally safe, trauma-informed care while fostering team collaboration and program innovation.

JOB SUMMARY:

The Primary Care Co-Lead (RN) will work in an environment dedicated to Indigenous health and wellness and will provide strategic direction and oversight for the team of primary care providers. This team aims to provide culturally-safe and effective primary care for people of all nations that is both inclusionary and flexible for individuals with mental illness, substance use disorder, or concurrent disorders. Our goal is to meet the holistic needs of our members and to provide access to services and integration across the care continuum.
The Primary Care Co-Lead must embrace the “patient medical home+” model of care and support continuous quality improvement, clinical education, and research. The PC Co-Lead should embrace the core values of KL, which include: the promotion of equity & wellness, respect, kindness, and Indigenous cultures and knowledge. Under the guidance of this position, KL Nurses will facilitate the creation of best practice guidelines that the Primary Care team will work to uphold.
The Primary Care Co-Lead will be an active member of the KL Leads team, alongside the PC Co-Lead (Physician), the Relational Care Co-Leads, the Administrative Lead, the Allied Health Lead, and the Indigenous Health Lead. This role will also work closely with a multidisciplinary team of Physicians, Nurse Practitioners, Medical Office Assistants, Elders, Outreach Workers, Social Workers, and Counselors.
The PC Co-Lead is expected to collaborate/partner with the Executive Director/Indigenous Health Lead to create a working partnership model of care with Indigenous Elders and traditional Knowledge Keepers. The aim of this collaboration is to facilitate the inclusion of Indigenous health and wellness practices and approaches in the primary care setting.

QUALIFICATIONS & SKILLS:

  • Baccalaureate degree or diploma in Nursing. Preference will be given to candidates with Indigenous ancestry and to those with significant lived experience working with Indigenous people.
  • Current practicing registration with BC College of Nurses and Midwives (BCCNM) (with Certified Practice In reproductive health,and opioid use disorder a benefit)
  • Current CPR.
  • Minimum of 2 years experience in primary care, preferable within the DTES, or experience in a remote/ rural setting.
  • Minimum 2 years experience in a leadership role.
  • Follow the Professional, Practice and Scope of Practice Standards and competencies as outlined by BCCNM; commit to annual continuing education to meet competencies.
  • Expert knowledge of inner-city primary care, substance use disorders, and adult education principles, methods, and tools. A commitment to ongoing professional development in Indigenous Cultural Safety is an expectation.
  • Experience with substance use and OAT (Opioid agonist therapy)
  • A commitment to developing knowledge and relationships that will support the facilitation of patient access to Indigenous healing modalities; knowledge of the social, political, and historical impacts of colonization on the health and well-being of Indigenous Peoples; knowledge of legislation and government policies and procedures that impact Indigenous Peoples’ health; and a broad knowledge of Indigenous cultures within North America.
  • Leadership and management skills to effectively support the Primary Care team to achieve its aims and objectives.
  • Ability to work with individuals with diverse backgrounds, adjust to new or unexpected events, the skills and experience to deal with and/or guide others in resolution of conflicts, and a willingness to provide consultation, leadership, and work direction.
  • Flexible, team-oriented, and committed to respectful, collaborative problem solving with team members.
  • Demonstrated commitment to culturally-safe, trauma-informed, member-centered, harm reduction and recovery-oriented practices.
