We are seeking a dedicated and compassionate Nurse Practitioner to join our Primary Care team. In this role, you will provide comprehensive, patient-centered care to individuals across the lifespan, focusing on health promotion, disease prevention, and management of chronic conditions. Job Responsibilities • Provide comprehensive primary care services, including routine exams, wellness visits, and management of chronic and acute conditions. • Conduct thorough patient assessments, order and interpret diagnostic tests, and develop individualized treatment plans. • Prescribe and manage medications in accordance with evidence-based guidelines. • Provide patient education and counseling on disease prevention, lifestyle modification, and health maintenance. • Collaborate with physicians and multidisciplinary team members to ensure coordinated and effective care. • Maintain accurate and timely documentation in the electronic medical record (EMR). • Participate in quality improvement initiatives and support population health goals. Qualifications Education: • Master’s degree in Nursing (MSN) with certification as a Nurse Practitioner from an accredited program.
