Location: California (Multiple Locations Available)
$250,000–$280,000 base + quarterly bonuses
Monday–Friday, 8am–5pm | No Weekends | Phone-only On-Call Rotation
$10,000 Sign-On Bonus + Up to $50,000 Relocation Assistance + Equity Grants

This is an opportunity to provide holistic, preventative, and longitudinal care for your patients — not just chase symptoms. KEY RESPONSIBILITIES:

• Manage a panel of ~150 participants with a focus on whole-person primary care
• Conduct comprehensive geriatric assessments and guide IDT care planning
• Participate in IDT meetings, coordinating care plans with social workers, therapists, and nurses
• Provide oversight for NP care delivery and coordinate use of specialists
• Participate in a low-volume phone-only call rotation (nights/weekends)
• Make occasional home visits and oversee care in nursing homes as needed
• Review clinical outcomes and hospitalization data to improve care quality
