Responsibilities

The physician will provide comprehensive primary care to older adult patients, maintaining full accountability for clinical outcomes and patient experience within a value-based Medicare Advantage model. This includes serving as a health coach, coordinating specialty care, leading the interdisciplinary team, and supervising Advanced Practice Providers.
