What Makes This Role Different Value-based, full-risk Medicare Advantage model Patient panel capped at ~450 patients Focus on prevention, care coordination, and behavior change Dedicated care teams (MA, Care Coordinator, Care Facilitator) Emphasis on quality, patient experience, and physician wellness Your Role Provide comprehensive primary care to older adult patients Maintain full accountability for clinical outcomes and patient experience Serve as a health coach, guiding lifestyle and long-term health improvement Coordinate specialty and hospital care while staying actively involved Lead and collaborate with an interdisciplinary care team Supervise and remain accountable for Advanced Practice Providers (APPs) Partner with medical and operational leadership to meet quality and cost goals Performance Focus (Supported & Measured) Hospital admissions per thousand Appointment utilization (18–21 slots per day) Patient experience (CG-CAHPS) Closure of clinical care gaps Medical cost management Qualifications MD or DO required Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics Active or eligible state medical license Active DEA Commitment to value-based, team-oriented care Compensation & Benefits Salary: $250,000 – $306,714 (based on experience) Comprehensive benefits package Paid time off and CME Long-term career growth and leadership opportunities Predictable schedule and strong work-life balance #MASC102
