This is a value-based care model focused on quality outcomes, preventive medicine, and delivering an exceptional patient experience to Medicare populations. POSITION OVERVIEW The Primary Care Physician provides comprehensive outpatient care within a team-based, value-driven environment. Physicians are supported by dedicated care teams including scribes and medical assistants, allowing focus on patient outcomes rather than volume-based metrics.
