This is a value-based care model focused on quality outcomes, preventive medicine, and delivering an exceptional patient experience to Medicare populations. POSITION OVERVIEW The Primary Care Physician provides comprehensive outpatient care within a team-based, value-driven environment. Physicians are supported by dedicated care teams, allowing focus on patient outcomes rather than volume-based metrics. SCHEDULE • Full-time, 40 hours per week • Outpatient setting COMPENSATION • Base Salary Range: $174,070 – $374,920 • Value-based bonus structure tied to patient outcomes • Compensation varies based on experience, education, and geography KEY RESPONSIBILITIES • Assess, diagnose, and treat patients in an outpatient primary care setting • Provide comprehensive care for Medicare patient populations • Prescribe medications and develop treatment plans • Maintain accurate electronic medical records • Collaborate with interdisciplinary care teams and leadership • Support quality initiatives focused on preventive care and reduced hospitalizations • Lead and guide care team members in clinical decision-making WHY THIS OPPORTUNITY • Value-based care model focused on quality over volume • Strong clinical support teams allowing physicians to focus on patient care • Opportunity for leadership track advancement • Mission-driven organization improving care for underserved populations • Stable outpatient environment with predictable schedule CARE MODEL • Value-based primary care • Medicare patient population • Team-based collaborative care approach • Focus on preventive medicine and improved outcomes Requirements • MD or DO required • Board Certified or Board Eligible in Internal Medicine or Family Medicine • Active, unrestricted state medical license required • Active DEA license required • US work authorization required • Experience in geriatric care preferred • Experience in value-based care preferred • Outpatient primary care experience preferred • Ability to work in a collaborative, team-based environment • Bilingual skills a plus (not required) Not ready to formally apply?
