We welcome physicians with Value-Based Care (VBC) experience as well as those eager to learn. Responsibilities
Deliver exceptional preventive and chronic disease primary care for adults aged 65 and older. Work within an integrated care team to manage a daily census of 12-15 patients with extended visit lengths.
