We are seeking a dedicated Primary Care Physician (PCP) to join our growing team in Montgomery following a physician transition. This is an opportunity to practice meaningful medicine with the time, support, and resources needed to truly impact patients’ lives. Our care model is designed around smaller patient panels, interdisciplinary collaboration, and proactive wellness-focused care for Medicare-eligible adults and underserved senior communities. Physicians are empowered to build lasting relationships with patients while supported by an experienced clinical and operational team committed to improving outcomes, reducing barriers to care, and strengthening the communities we serve.
