We are seeking a dedicated Primary Care Physician (PCP) to join our growing team in Baltimore following a physician transition. This is an opportunity to practice meaningful medicine with the time, support, and resources needed to truly impact patients’ lives. Our care model is designed around smaller patient panels, interdisciplinary collaboration, and proactive wellness-focused care for Medicare-eligible adults and underserved senior communities. Physicians are empowered to build lasting relationships with patients while supported by an experienced clinical and operational team committed to improving outcomes, reducing barriers to care, and strengthening the communities we serve. Compensation & Benefits Base Salary ranging from $250,000 for new graduates/residents up to $295,000 for experienced physicians Annual Incentive Bonus with achievable performance goals and recent physician payouts ranging from $45,000 to $85,000 $15,000 Sign-On Bonus Medical, Dental, and Vision Insurance Generous PTO and CME allowance Short- & Long-Term Disability Coverage Life Insurance equal to one year of base salary Malpractice coverage including tail coverage Practice Highlights Smaller patient panels that allow more meaningful patient interactions Value-based care model focused on prevention, wellness, and coordinated care Modern, technology-enabled practice environment On-site diagnostics including labs, EKG, and echocardiogram Collaborative interdisciplinary care team including nurses, MAs, case managers, social workers, and community outreach specialists Dedicated support for care coordination, patient navigation, and population health management Continuous professional development and training in value-based care Qualifications MD or DO from an accredited medical school Board-certified or board-eligible in Internal Medicine or Family Medicine Active or eligible Maryland medical license Technologically savvy physician strongly preferred Experience in value-based care is a significant plus Why Join Us?
