Description Hours for Position: 8:00 AM – 5:00 PM Weekdays (M-F) On-Call Rotation (Every 6 to 8 Weeks) POSITION SUMMARY: The Primary Care RN a hands-on clinic position, with responsibility for assessing, coordinating, monitoring, and providing health care services and treatment for the PACE program participants. The position ensures provision of high-quality care, including coordination with the primary care team. PRINCIPAL DUTIES AND RESPONSIBILITIES: The following represents most of the duties performed by the position but is not meant to be all-inclusive or prevent other duties from being assigned when necessary: · Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, unscheduled, and annual assessments; attend morning updates and report changes in participants’ baseline status to appropriate staff on a daily basis. · Involved in the development and implementation of QAPI activities. · Medication management in conjunction with PACE and specialty care providers. Ensures LTC MARS and PACE Medications are reconciled monthly and as needed. · Maintain accurate, detailed reports and records. Record patients' medical information and vital signs. assessing their conditions and needs in order to develop personalized care plans. · Communicates change in condition or concerns immediately to team · Providing case management oversight and coordination of plan of care. · Demonstrates a thorough understanding of HIPAA/HITECH laws and the related policies and procedures within the agency. Attends and successfully completes mandatory HIPAA/HITECH training at least annually. · Provide accurate and timely utilization and LIFEPACE data. · Provide on-call coverage during center hours of non-operation. · Maintain confidentiality regarding participant, staff, contractor, and organizational information. · Attend and participate in interdisciplinary team meetings, staff meetings, in-services, projects, and committees as assigned. · Accept assigned duties in a cooperative manner and perform all other related duties as assigned. · Be flexible in schedule of hours worked. Requirements CERTIFICATION, LICENSES, REGISTRATIONS: · Hold and maintains current Registered Nurse license with the State of Oklahoma. EDUCATION AND EXPERIENCE: · Minimum of two (2) years of demonstrated clinical nursing experience; prefer wound care experience. · Minimum of one (1) year of documented experience working with a frail or elderly population. · Experience in the provision of nursing care, including assessment skills, health education, and current nursing treatments and protocols. KNOWLEDGE, SKILLS, AND ABILITIES: · Working knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families. · Effective management skills. · Knowledge of utilization review, quality improvement and cost containment systems. · Ability to work with ethnically diverse populations. · Ability to apply creative problem-solving skills to the complex issues in home care. · Ability to work with interdisciplinary teams. · Strong organizational skills, flexible, resourceful, and energetic. · Effective oral and written communication skills. PHYSICAL REQUIREMENTS: Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead, squatting and kneeling. Ability to communicate verbally, with an excellent comprehension of the English language. WORK ENVIRONMENT: The main work environment is a medical clinic setting of a non-profit organization providing a variety of services to seniors and their families. The activity level is high and the noise level is moderate. The work environment will include a variety of office, clinician and day centers settings.
