We have an exciting opportunity to join our NHS Primary Care team at Havant and East Hants Mind as a Primary Care Wellbeing Advisor.
The Primary Care Wellbeing Advisor is based within GP surgeries and they assess patients on an individual basis in order understand their mental health and to identify the most appropriate measures to help support them.
Using a collaborative and integrative approach, this service greatly enhances the support GP practices offer to their patients, and is an essential role to ensure that patients are receiving the care they need.
We are looking for a caring, empathetic individual with the required skills and knowledge outlined in the recruitment pack, to join our team. It is important you are able to manage your own workload and work independently, but also able to work collaboratively within the wider PCN team.
The salary is £25,992.05 per annum, which is £15,805.98 pro rata for 22.5 hours per week.
