Description Job Description: The Care Coordinator at Next Level Medical plays a vital role in supporting our healthcare team by managing and coordinating various aspects of patient care. Reporting to the Care Navigator Manager, this position helps to ensure seamless communication, appointment scheduling, and record-keeping for our patients, all while upholding our core values of Service-Excellence, Teamwork, Integrity, Innovation, and Compassion. Responsibilities: Serve as a primary point of contact for patients, ensuring timely and appropriate communication throughout their healthcare journey Collaborate with Care Navigators, physicians, nurses, and other healthcare providers to facilitate the coordination of patient care Schedule appointments, tests, and procedures, and assist with insurance authorizations and referrals as needed Maintain accurate and up-to-date patient records, documenting all care coordination activities and interventions Provide administrative support to the Care Navigator team, including data entry, report generation, and other tasks as required Participate in ongoing professional development and training to stay current with best practices in care coordination and patient support Pay Range: The salary range for the Care Coordinator position is $35,000 - $45,000 per year, commensurate with experience and qualifications.
