Salary: $37.50 - $41.50 / per hour Maxim Healthcare is hiring for a Registered Nurse (RN) to work on our Transition team. Our Transition Team is a supplemental care unit of skilled nurses whose primary function is to quickly and efficiently get patients home while primary nursing staff are actively recruited. This team is deployed after we receive a patient referral for home care services and the patient is admitted to Maxim. Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-90 days. During this time, our office staff will be recruiting to identify full-time, replacement home healthcare nurses. The new caregivers will have an opportunity to train and shadow in the home with the Transition Team caregivers prior to working independently with the patient. Once the replacement nurses are trained and have a schedule, the Transition Team are then transitioned to another new admission. Why Join Maxim? Responsibilities: Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable Requirements: Current Registered Nurse (RN) License for the state in which the nurse practices. Current Health Certificate (per facility Registered Nurse (RN) contract or state regulation). Current PPD or Chest X-Ray. Current BLS card. One year prior Registered Nurse (RN) experience preferred.
