As we continue to grow, we are seeking talented and motivated Patient Pathway Coordinators to join our dynamic team. Salary for this role is £23,874.88-£26,397.29 per annum WTE.

JOB DESCRIPTION

  • Acting as the main point of contact for patients receiving private outpatient care; providing support, information and guidance throughout the patient’s journey.
  • Co-ordinate, monitor and initiate appropriate changes within administrative processes along the patient pathway to ensure the patients journey is managed efficiently, smoothly and in accordance with targets and agreed timescales.
  • Efficiently scheduling appointments for tests or procedures, ensuring timely and convenient arrangements for patients.
  • Developing expert knowledge of the private services clinical/booking system (Semble) and training other staff members in its use
  • Provide reception duties to support community services clinics, as required
  • Enter patients onto the clinical system, ensuring all demographic details are correct (addresses, phone numbers GP details etc.) updating correctly on clinical/data systems as necessary.
  • Undertaking the administration of registration, transfers and discharge of patients using the appropriate systems.
  • Participate in the validation of system data to ensure that the data held is up-to-date and accurate and recorded appropriately e.g. patient pathway audits.
  • Work flexibly during times of service need or staff absence, including occasional evening and weekend working.
  • Courteously and efficiently receive telephone enquiries from patients, carers, and external agencies in relation to appointments and general enquiries, ensuring that accurate records, both manual and electronic are maintained and appropriate action is taken in a timely manner.
  • Receive and process incoming correspondence, paper and electronic, ensuring it reaches the appropriate place within the required deadlines.

KNOWLEDGE:

  • Proven experience as a Patient Pathway Coordinator or a similar role
  • Proven experience in administrative roles, preferably within a healthcare or private services setting.
  • Knowledge of full range of administrative procedures and associated equipment.
  • Knowledge of software programmes including Microsoft Office Word, Excel, Outlook and other Private Outpatient Systems.
  • Self-motivated and ability to work without supervision when required.
  • Excellent organisational and communication skills.
  • Ability to maintain computerised and manual filing/documentation systems.
  • Understanding of how clinical services work and the importance of/inter-links between the duties commensurate with this role.
  • Previous experience of working in a clinical provider organisation.
  • Knowledge/previous experience of clinical system.

SKILLS:

  • To be able to write clear, concise messages often involving complex medical terminology.
  • To understand a range of Protocols and Policies and implement them in everyday work.
  • Good standard of written communication.
  • Excellent communication and interpersonal skills to interact with patients, healthcare professionals and internal staff.
  • To comprehend complex instructions or documents containing medical terminology.
  • Highly developed customer service skills and ability to provide and facilitate a culture of exceptional customer experience to Modality Clients.
  • To be able to communicate with all levels of personnel, including medical professionals, patients and outside agencies.
  • To be able to relay clear, concise messages containing medical terminology without risk of misunderstanding.
Responsibilities

As the Private Services Patient Pathway Coordinator, you will play a crucial role in ensuring a seamless and efficient journey to our patients from initial consultation through to post treatment care where you will be responsible for patient liaising, coordinating and managing the patient pathway, collaborating with various departments to optimise the delivery of our private outpatient services along with completing a wide array of administrative tasks. The Patient Pathway Coordinator will support the Clinical Team, Team Leader and other Managers in ensuring that a variety of specialities are able to deliver safe and effective care whilst also ensuring compliance and other KPI targets are completed within a set timeframe.
