Description The Home Health Occupational Therapist provides expert therapeutic services to patients in their homes, focusing on enhancing their ability to perform daily activities and improving their overall quality of life. In this role you will assess, plan, and implement individualized therapy programs in collaboration with patients, families, and other healthcare professionals to address physical, cognitive, and emotional challenges. Your role is essential in helping patients regain independence and achieve their personal goals in a home environment. Schedule: PRN, Weekdays, 8-5 Primary Responsibilities and Duties: Conduct comprehensive assessments of patients' physical, cognitive, and emotional needs to develop personalized therapy plans. Design and implement individualized therapy programs to address impairments, functional limitations, and performance deficits. Educate patients and their families on techniques and strategies to improve daily living skills and adapt their environment to support functional independence. Utilize therapeutic interventions, including exercises, adaptive equipment, and modifications to daily activities, to enhance patients’ abilities and quality of life. Monitor and document patient progress, response to therapy, and any changes in condition, providing detailed reports to the supervising physician and other healthcare team members. Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure a holistic and coordinated approach to patient care. Provide guidance on the use of assistive devices and adaptive technologies to support patients’ functional abilities. Maintain accurate and up-to-date patient records and documentation in compliance with regulatory and organizational standards. Offer support and education to caregivers and family members to improve patient care and facilitate the therapy process. Participate in ongoing professional development and stay current with advancements in occupational therapy practices and techniques. Perform all other duties assigned. Requirements Job Qualifications: Valid Occupational Therapy license in the state where work is performed. 3 or more years of occupational therapy experience, preferably in a home health or similar setting. Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, caregivers, and healthcare professionals. Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment. Ability to build rapport with patients through a compassionate and caring demeanor. Physical Requirements: Ability to lift and transfer patients, with or without assistance, up to 50lbs. Capability to perform physical tasks such as bending, standing, walking, and climbing stairs for extended periods. Strength to handle therapeutic equipment and assist patients with mobility and physical activities. Visual acuity and dexterity for performing tasks such as reading medical records, administering injections, and handling therapeutic tools. Ability to respond swiftly and efficiently to emergency situations. Capacity to work in various home environments which may include navigating stairs, tight spaces, or uneven surfaces. Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager. Job requires accepting criticism and dealing calmly and effectively with high-stress situations. The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
