Description The PRN On Call Nurse provides skilled nursing assessment (within scope of practice) and care to terminally ill patients in the Hospice of Hope service area after normal business hours and on weekends as scheduled. Provides supportive nursing care to patients and families in time of need for increased services. Meets referred patients to educate on the benefits of hospice care and to complete the admission process. Duties include: Use clinical tools, data and other information to complete assessments of patient needs Provides care in accordance with the individualized plan of care Educate patients and caregivers on end-of-life, services and other resources Document complex medical information Follow quality care processes in compliance with regulatory guidelines and company procedures Strong written and verbal communication skills with patients, physicians and other stakeholders Demonstrated ability to assess and respond to patient and family needs in a timely manner Basic computer skills, including working knowledge of the Microsoft Office suite Experience in establishing professional and collaborative relationships with patients, families and providers Evening or Night Shift: 4:00 pm to 12:00 am, 12:00 am to 8:00 am, with weekends and holidays as scheduled Care Site - private residences, nursing homes, assisted living and other facilities Requirements Graduate of an accredited School of Nursing with current licensure in Kentucky and/or Ohio. At least one year of experience in a patient care position in a hospital, nursing home, or related health care facility preferred. Must carry state minimum in automobile liability insurance. Must arrange travel and be able to access client homes, nursing homes, hospitals, and other facilities independently. Must demonstrate proper use of personal protective equipment and observe universal precautions. Must be able to lift at least 50 pounds using proper body mechanics. Must have valid driver's license, current auto insurance and reliable transportation.
Responsibilities
