Job Summary: The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician. Responsibilities/Requirements: Screen all residents in facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Physical Therapy intervention is indicated. Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual. Reassess patients and caseload at least every 30 days to determine the need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated. Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated. Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment. Educate staff, family, and care givers as indicated. Identify needs for program development and implement programs according to company policies and procedures. Identify equipment needs of Physical Therapy Dept. and order equipment according to company and facility policy. Complete all required documentation according to guidelines specified in Documentation Manual. Coordinate in-services for facility staff as need is determined, to ensure safe and effective carry over of Maintenance/Restorative Programs and overall call of the residents. Attend all facility clinical required meetings (Medicare, Care Plans, etc.). Promote profession within facility, including assisting with coordinating special events i.e. National Physical Therapy Month. Attend Continuing Education courses specific to current position. Maintain professionalism and always follow all national/state associations regulations. Supervise student affiliates as directed. Assist employees in the absence of Clinical Manager. Qualifications Qualifications: Bachelor's, Master’s, or Doctorate in physical therapy from an accredited program. Certification and licensure requirements as established by state and national board associations. Active state license required. Excellent organizational and time management skills Excellent written and verbal communication skills Possess current working knowledge of Functional Pathways policies and procedures, and clinical programs with application skills to therapy disciplines. Ability to supervise Physical Therapy Assistants Completion of all required screenings, certifications, and licensure.
