Overture Home Care is currently seeking a dynamic and skilled RN to join our team! We are currently looking for someone available to work PRN and assist with client assessments as needed. We assist seniors in and around the Dallas area, and specialize in Alzheimer's / Dementia care, hospice, and general geriatric care. Responsibilities and Duties Conduct in-home and/or facility-based client assessments as needed to determine care needs and eligibility Collaborate with intake, case management, and nursing teams to ensure seamless care coordination Evaluate client conditions and recommend appropriate levels of care and services Maintain accurate and timely documentation of all assessments and clinical findings Communicate effectively with clients, families, and referral sources to support care planning Ensure compliance with all state regulations and company policies during assessments Support care transitions from hospitals, rehab facilities, or other care settings Build and maintain professional relationships with referral partners and healthcare providers Provide clinical insight and support nursing and caregiving staff as needed Participate in case reviews and contribute to ongoing quality improvement initiatives Travel within the Dallas area to complete assessments as required Schedule PRN (as-needed) position with flexible scheduling Hours will vary based on client assessment needs and referral volume Opportunity to pick up shifts during weekdays, evenings, and occasional weekends Ideal for individuals seeking supplemental income or flexible work alongside another role Advance notice will be provided when possible; some same-day or next-day availability may be requested Requirements Must have a full, active license as a Registered Nurse. Minimum of two years related work experience in a geriatric healthcare setting required. Flexibility as needed when it comes to assessments and scheduling trainings Demonstrates effective verbal and written communication skills Ability to work independently Demonstrates initiative, problem solving ability, adaptability and flexibility Clinical skills: documentation of care, medication administration, infection control precautions, assessing patient educational needs, physical assessment, obtaining vital signs, care coordination.
