What You’ll Do: Provide comprehensive care management and coordinate patient care plans across various settings. Serve as the key advocate for your patients, ensuring they receive the best possible care. Conduct patient assessments, develop personalized care plans, and adjust treatment as necessary. Provides health care instructions to the patient as appropriate per assessment and plan of care. Monitor patient progress and outcomes to continuously improve care quality and patient satisfaction. Educate patients and families on care plans, treatments, and necessary follow-ups. This is position allows for work in the field and requires regular weekly travel to client homes throughout WAKE County and surrounding areas*. Perform supervisory visits every 90 days (60 days if CAP) for clients in assigned areas. Communicate any changes in client condition with primary nurse. Communicate any concerns regarding aides with field supervisor. Regularly re-evaluates patient nursing needs. Requirements Active RN license in North Carolina or have a multi-state license. Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma. Minimum of 2 years of nursing experience (case management experience preferred). Demonstrated ability to read, write, and effectively communicate in English. Strong clinical skills and a deep understanding of case management principles. Excellent communication and patient advocacy skills. Prior home care experience strongly preferred, but not required. A passion for helping others and a compassionate, patient-centered approach to care. Solid computer skills: prior experience with electronic medical records (EMR) preferred. Must have own reliable vehicle Must be flexible Ability to travel to cases as assigned.

Responsibilities

Coordinate comprehensive care plans and serve as a primary advocate for patients in a home care setting. Conduct regular assessments, monitor patient progress, and provide health care instructions to patients and their families.
