We are looking for a passionate PRN Social Worker to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.    Position Summary: Primary function is to provide coordinated care in the homes to patients of all age groups and perform psychosocial assessments, analysis, counseling, and referrals to meet the needs of the patient/family. Qualifications: * Bachelors or master’s degree in social work from a State accredited program. If has a bachelor’s degree and is hired after 12/02/2008, must be supervised by a Master of Social Work. * One (1) year social worker experience in a health care agency. * Working knowledge of community resources. Good interpersonal skills. Hepatitis profile. * Current State Certification * Reliable transportation.
