Description Summary The Registered Nurse serves as a case coordinator by providing, arranging for, teaching and supervising the health care of hospice patients in their home. The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team. Requirements Qualifications/Education: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Licensure: Must have and maintain license as a registered nurse by the state of Indiana. Minimum of associate degree in nursing, graduate of a school of professional nursing. 2. Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or home came preferred. Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred. Must be sensitive to issues of loss and grief. 3. Must maintain current CPR certification. 4. Must hold and maintain a valid Indiana driver’s license and auto insurance. Knowledge and Ability: 1. Demonstrate comprehensive knowledge and skill in current nursing practice. 2. Updates nursing knowledge and skills through attendance at in-services, continuing education, and reading nursing journals and periodicals. 3. Demonstrates understanding of and ability to articulate agency philosophy and concepts. 4. Demonstrates knowledge on the agency’s programs and the qualifications patients need to utilize these services. 5. Ability to use and adapt to current technology. 6. Responsive to supervision and asks for help when needed. 7. Effectively utilizes problem-solving approach to patient care and record-keeping. 8. Functions independently and appropriately and demonstrates the ability to deal effectively with the demands of the job. 9. Ability to accept responsibility under adverse working conditions, within limits of safety, such as inclement weather conditions, substandard living conditions of patients, etc. 10. Ability to work independently, exercising appropriate judgment consistently. 11. Ability to work in a fast-paced environment responding quickly and exhibiting high energy and a calm methodical approach. 12. Working knowledge of Medicare/Medicaid guidelines and ability to interpret agency, state, and federal regulations. 13. Ability to deal effectively in stressful situations; demonstrates good conflict resolution skills. 14. Must be flexible and adapt easily to a changing environment. Ability to multi-task easily.
