Location: Village on the Park Onion Creek, and Independent Living and Assisted Living senior community  Schedule: Must be available on weekends and holidays Essential Duties and Responsibilities: •    Administer medications safely and accurately per physician orders and regulatory standards •    Oversee medication aides and conduct monthly medication reconciliation and audits •    Coordinate daily staff assignments to ensure consistent resident care •    Communicate effectively with physicians, families, and care teams regarding resident needs •    Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence support •    Respond to medical and behavioral emergencies with sound clinical judgment •    Complete clinical documentation including assessments, evaluations, and service plans •    Document incidents and communicate promptly with leadership •    Monitor care team documentation for accuracy and completeness •    Support orientation and ongoing training of care staff •    Assist with scheduling in collaboration with leadership •    Attend required in-services and continuing education •    Maintain professional appearance and hygiene-  •    Promote respectful and effective communication across teams and families\ •    Perform other duties assigned to support resident care and operational excellence Qualifications: •    Current and valid LVN/LPN license in the state of employment •    Basic Life Support (BLS) / CPR certification •    Minimum 6 months to 1 year experience in Assisted Living or Long-Term Care •    Proficient in Electronic Health Records (EHR) and basic computer skills •    Strong verbal and written communication skills •    Demonstrated critical thinking and problem-solving abilities •    Knowledge of HIPAA regulations and compliance standards •    Ability to respond with patience and understanding to dementia-related behaviors •    Physical ability to perform essential job functions with or without reasonable accommodation EOE/M/F/D/V
