Schedule: Monday – Friday, 8:30 AM – 5:00 PM, some evenings, some holidays and weekends as scheduled.
Work Location: Off-site at referral source locations in Montgomery and Prince George’s County as advised by manager.

POSITION SUMMARY:

The Professional Liaison is responsible for generating qualified hospice referrals to meet or exceed quarterly marketing and admissions goals. This field-based role involves frequent off-site engagement with physicians, hospitals, skilled nursing facilities, assisted living communities, and other potential referral sources. Through scheduled visits, in-services, and presentations, Liaison educates healthcare professionals and community partners about hospice services to drive referrals and support organizational growth.
The Liaison works collaboratively with clinical teams to ensure smooth and timely transitions for referred patients, serving as a key point of contact between the referral source, the patient, and the hospice team. They are responsible for accurately tracking referral activity and maintaining up-to-date records in the customer relationship management (CRM) system. Additionally, the Liaison conducts ongoing market analysis to identify referral trends, competitive activity, and growth opportunities. Participation in community events, health fairs, and outreach initiatives is also expected to raise awareness and visibility of hospice services.

MINIMUM QUALIFICATIONS:

  • Bachelor’s degree in related field preferred
  • Minimum 2 years of experience in healthcare sales or hospice/ home health marketing.
  • Knowledge of Medicare, hospice benefit and endoflife care are strongly preferred.
  • Excellent interpersonal, communication, and presentation skills.
  • Selfmotivated with strong organizational and time management abilities
  • Valid driver’s license, clean driving record and reliable, insured transportation required.
  • Requires mobility to meet demands of outside appointment schedule, including driving within metropolitan area, accessing various offices and medical facilities.
  • Must be able to spend a minimum of 75% of time offsite on salesrelated activities.
  • Required to work some evenings as needed and some holidays and weekends as assigned.

Responsibilities
  • Drives qualified hospice referrals through direct, inperson engagement with physicians and care providers to meet or exceed quarterly sales targets and contribute to organizational revenue growth.
  • Spends a minimum of 75% of time offsite engaging with patients, families, and referral sources to educate them on Montgomery Hospice’s programs, services, and philosophy of care.
  • Meets regularly with physicians to solicit and secure hospice service orders and strengthen referral relationships
  • Keeps current on knowledge of hospice benefits covered by Medicare and other insurers.
  • Provides input for written material such as newsletters and brochures.
  • Coordinates referrals to both Hospice at Home and Casey House.
  • Meets with physicians, hospital discharge planners, nursing home social workers, and liaisons to assure services are meeting needs of referred patients and families.
  • Attends discharge planning rounds, educational programs, case conferences and staff meetings at hospitals as appropriate.
