In addition, Yee Hong offers four campuses of managed care, a hospice residence, and a wide range of community-based senior and caregiver support services. QUALIFICATIONS:

  • Diploma in Social Work or Social Services, health sciences, activation, or equivalent
  • Experience in a community/social services work setting preferred
  • Knowledge of the social welfare system, social service provision for seniors in the community
  • Recent certification in C.P.R.
  • Ontario driving license and access to an automobile an asset
  • Fluency in English and ability to converse in Chinese
  • Must be able to work across different Yee Hong sites as needed
Responsibilities
  • Under the overall direction of the Program Manager/Program Coordinator, participates in the planning of appropriate activities for clients of the program
  • Implements and ensures smooth operation of the activities in a safe, hygienic and supportive environment
  • Provides timely and appropriate crisis intervention or behaviour management and ensures that the clients’ physical, social and emotional needs are appropriately met
  • Provides timely and appropriate support/information and referral/placement/outreach to clients/volunteers (as appropriate) to ensure that the clients/volunteers’ social/ recreational/development needs are appropriately met
  • Works closely with client and caregiver in ensuring appropriate design of care plan/program plan and ongoing monitoring of plan
  • Ensures appropriate assessment, placement, matching and support for volunteers
  • Provides effective guidance, training and support to the volunteers in the program
  • Maintains up to date and accurate files, records and statistics on the program
  • Maintains accurate record of fees and expenditures
  • Complies with risk management policy for health and safety and infection control
  • Keeps up-to-date with developments and best practices in the field
  • Other duties as assigned
