Youth Wellness Hubs Ontario (YWHO, www.youthhubs.ca) is a network of 32, Integrated Youth Services (IYS) hub networks that currently operate in 40 communities, where young people ages 12 to 25 years have walk-in access to youth-centred, community-based mental health and wellness services. YWHO engages youth, family members, service providers, and other partners through a co-creation process, enhancing the relevance, acceptability and youth friendliness of services. The hubs are provincially-consistent in core features and locally led and adapted to offer and connect to a range of evidence-based services—mental health care, substance use health services, primary care, education, employment, housing, peer support, family support and care navigation—in youth-friendly spaces. Outcomes are rigorously evaluated within and across hubs, focusing on positive outcomes in areas of wellness important to youth and their families. This position will be located in Ontario, anywhere outside of the Greater Toronto Area.

POSITION OVERVIEW

Youth Wellness Hubs Ontario Provincial Office (YWHO PO) is recruiting a full-time, contract (12 months) Program Consultant (Integration and Implementation Specialist, YWHO Virtual Services). Reporting to the Manager of Virtual Services, the Integration and Implementation Specialist will support the expansion of the YWHO Virtual Hub and Virtual Network, including the establishment of a Provincial Youth Wellness Team (YWT), dedicated to providing virtual care to youth across Ontario. This role focuses on implementation support of the YWHO Model within the YWHO Virtual Hub at both the provincial and local levels, as well as providing implementation support to a number of Hub Networks. The Integration and Implementation Specialist will collaborate across multiple teams at the YWHO PO—including Knowledge Mobilization, Data and Evaluation, Equity, Engagement, and Research—as well as YWHO Hub Networks, youth and family members and additional community partners to ensure successful adoption, integration, and enhancement of the YWHO Virtual Hub.

Job Requirements

  • Master’s Degree in relevant discipline (e.g. Health, Education, Social Services, or a related field).
  • Minimum 3 years of experience in mental health, substance use, or health services.
  • Experience implementing digital health solutions, virtual service delivery, or technology systems integration.
  • Strong background in implementation science, quality improvement, and change management.
  • Experience in community-based health initiatives with youth and families.
  • Demonstrated ability to engage diverse communities and promote equity in service delivery.
  • Familiarity with provincial and local mental health and substance use systems.
  • Proficiency in Microsoft Office including Word, Excel, and PowerPoint.
  • Excellent communication, facilitation, and problem-solving skills.
  • Ability to travel occasionally for implementation support, with a valid driver’s license and access to a reliable vehicle.
  • Ability to work independently as well as collaboratively with a diverse team, demonstrating strong interpersonal and community engagement skills.
  • Bilingualism (French/English) or proficiency in an additional language is an asset.
  • Reside outside of the GTA
Responsibilities
  • Applying evidence-informed approaches to implement and enhance the YWHO Virtual service delivery model.
  • Supporting teams in adopting the YWHO Virtual Hub through facilitation, training support, and problem solving.
  • Assisting in system and practice changes using implementation science, quality improvement frameworks, and change management strategies.
  • Developing implementation plans, tools, and processes tailored to contextualized community needs.
  • Providing expertise on integrating equity, anti-racism, and anti-oppression principles into virtual care practices.
  • Collaborating with partners to build a shared vision for youth-centered virtual care.
  • Engaging with diverse partners and collaborators, including youth, families, and service providers, to ensure culturally responsive implementation.
  • Collaborating with the data and evaluation team to analyze service data to identify trends, successes, and opportunities for improvement.
  • Supporting the development and implementation of enhancements to the YWHO Virtual Hub and virtual service delivery model.
  • Coaching and guiding organizations in building capacity for virtual service delivery.
