Responsibilities

THE ROLE

The Program Coordinator is a pivotal role within Your LINC North, located in Elizabeth Park, serving the Northen Adelaide Local Health Network (LHN), NALHN & BHFLHN. The position oversees two key support programs - Intensive Crisis Support (ICS) & Non-acute Psychosocial Support (NAPS
Reporting to the Your LINC North Manager, the Program Coordinator is responsible for ensuring high-quality, responsive service delivery and managing a client caseload. You will monitor risk and quality management systems, attend case conferences, mentor staff, and maintain strong relationships with clients and stakeholders.

The following key responsibilities will ensure you contribute to the program meeting its objectives.

  • Monitor and coordinate incoming referrals to ensure suitable client-staff matching.
  • Participate in case conferences and client reviews, supporting team members, clients, and networks.
  • Maintain a client caseload, delivering individual or group support.
  • Ensure accurate and up-to-date case notes and documentation.
  • Mentor and inspire best practices within the team, promoting a positive and productive work environment.
  • Build strong relationships with key stakeholders.
  • Support site resource development and review to meet operational needs.
