As part of the Foster Success team, the Coordinator - Health & Well-being, advances the organization’s mission by ensuring teens and young adults have access to a continuum of Health & Well-Being programs and resources as they transition out of foster care. The coordinator serves as the point of contact for young people seeking support & resource connections and works with the Assistant Director of Health & Well-being to ensure the smooth functioning of program operations and activities including the Basic Needs program, the Mental health Acceptance, Promotion, and Support Initiative (M.A.P.S.), Vital Documents, “Real Adulting” workshops, and the Insuring Foster Youth Trust Fund. This position is responsible for connecting participants to a spectrum of resources, knowledge, and skills that enhance their overall well-being. The Coordinator- Health & Well-being will support in cultivating a culture of well-being across Foster Success’ impact areas among our mission population.
The Health and Well-being Coordinator reports to the Assistant Director of Health & Well-being.
This position will begin full-time, in-office (based in Indianapolis), and will have an opportunity to become a hybrid or fully remote position after onboarding.

PARTICIPANT SUPPORT



    • Complete Basic Needs and M.A.P.S. Intake processes and serve as a point of contact for young people seeking support through 1:1 referrals and connections to community resources in areas including but not limited to: Housing, Financial Support, Medical & Health Needs, Self-Advocacy, and Basic Needs (i.e. food, hygiene items, etc.). Empower young people in building long-term support networks. Engage in skill-building and relationship building work as necessary to fully support each young person in their well-being needs. Assist in planning and coordinating quarterly “Real Adulting” workshops on various topics, in collaboration with colleagues, community partners, and young people.

    POSITION REQUIREMENTS

    • Bachelor’s degree with 2 years of experience working with individuals affected by foster care, individuals from diverse backgrounds, or involved in the justice system
    • Demonstrated success in the areas of program implementation, service coordination, and attention to detail,
    • Experience working with transition aged youth between the ages of 14-26 from diverse backgrounds.
    • Experience facilitating learning and activities for youth or young adults
    • Mental health knowledge/experience is preferred,
    • Ability to represent Foster Success with senior-level partners and agencies
