Description

Leading social services organization seeks a Program Coordinator to ensure quality care, smooth resident transitions, and full compliance with state, federal, and CARF standards. The Program Coordinator partners with internal teams, families, and external stakeholders to deliver person-centered, compliant, and audit-ready services.

QUALIFICATIONS:

  • Master’s degree in a relevant field, or advanced certification in healthcare management/quality improvement.
  • Minimum 5 years’ experience with individuals with intellectual/developmental disabilities (IDD).
  • At least 2 years in a supervisory, compliance, or quality role.
  • Strong knowledge of OPWDD and DOH regulations.
  • Excellent communication, analytical, and problem-solving skills.

Responsibilities
  • Ensure IRAs/ICFs remain in full compliance with OPWDD, DOH, CARF, and federal regulations; maintain audit readiness.
  • Conduct chart and documentation reviews, track data, and report outcomes on a weekly basis.
  • Lead and monitor Plans of Corrective Action (POCAs) in response to internal and external audits.
  • Coordinate and oversee resident transitions, including admissions, discharges, and hospital/rehab returns.
  • Serve as co-facilitator of the Admission/Discharge Committee and manage all required documentation.
  • Act as the primary liaison with families, advocates, hospitals, and external partners to ensure smooth placement and continuity of care.
  • Track and ensure completion of required training for all IRA/ICF staff.
  • Support incident tracking, timely follow-up, and closure of recommendations.
  • Collaborate across disciplines as a member of the IRA/ICF management team, assisting with planning, problem-solving, and emergencies.
  • Contribute to system improvements using data-driven tools and provide coverage for colleagues as needed.
