Description Position Summary: The Program Coordinator of Kinship Care is responsible for coordinating all aspects of Kinship Care Programs, including home visits, working with case managers, maintaining documentation, managing files and other administrative processes. Pay:$20/hr. Essential Responsibilities: · Conduct quarterly home visits of all Kinship Care programs · Meet with family provider, review policies, walk through home (or conduct on phone), make recommendations if needed · Collect and process documentation for Kinship Care · Work closely with Case Managers; provide verbal and written communication on regular basis · Update care plans, update contracts, perform safety checks, track Progress Notes/ Fire Drills forms · Track Client/Provider changes in software · Forward updates of client status changes to Manager · Ensure quality of service in Kinship Care homes · Conduct home visits quarterly · Maintain up-to-date, accurate and complete client records in accordance with program policy · Assist Manager with program compliance · Teamwork, regular attendance in the office, and face-to-face interaction (as needed) are essential job functions · Other duties and responsibilities as assigned by supervisor Requirements Qualifications: Education and Experience · Bachelor’s degree preferred or the equivalent of 2 to 3 years of customer service and administrative experience · Proficiency in various software applications including Word, Excel, Access to promote efficient communication and accurate record keeping · $100,000/300,000 Automobile Liability Insurance · Valid NH driver’s license and access to reliable vehicle during working hours to transport individuals Skills and Abilities · Demonstrate a commitment to The Moore Center’s values · Understand and comply with all state, agency and department policies and procedures · Possess strong time management and organization skills, including the ability to prioritize multiple tasks and ensure timely follow through · Ability to work independently and as part of an inter-disciplinary team · Demonstrate ability to problem solve as necessary · Interact effectively with people of varied educational, socioeconomic and ethnic backgrounds, skill levels and value systems · Demonstrate the ability to encourage people · Demonstrate excellent verbal and written communication skills · Demonstrate a strong emphasis on customer service skills · Ability to interact professionally and courteously with referral sources and potential customers to facilitate successful customer relationships. · Ability to provide well-written, accurate and timely reports that ensure compliance with all regulations and established practices · Report suspected incidents of client abuse, neglect, and/or exploitation Note: Moore Center Services considers the ability to speak, read and write English essential functions of all positions in the best interest of the clients that we serve. Staff are also required to travel and transport clients when applicable. This includes having access to a reliable vehicle, a valid driver’s license and maintaining personal auto insurance of $100,000/300,000.
