Description

KNOWLEDGE:

· of human services, with a focus on services to older adults and/or individuals with disabilities;
· of various older adult and disabilities service delivery models;
· of non-profit and/or government procedures and reporting requirements;
· of spreadsheet and data processing computer applications;
· of basic digital communication and messaging applications for program promotion;

SKILL:

· in working cooperatively with managers and administrators of other agencies and organizations;
· in reading, interpreting, and effectively communicating regulations, policies, and program guidance;
· in the recruitment and retention of volunteers;
· in maintaining records and preparing and producing a variety of reports
· in effectively utilizing stand-alone and networked computers and data management systems to participate fully in day-to-day business and service delivery operations and generate reports;
· in providing effective community outreach and in both community and professional-level staff information, education and training provision.

TRAINING AND EXPERIENCE:

· Three years increasingly responsible experience in a professional setting and working with managers and administrators of other agencies and organizations, or,
· A.A., B.A., or equivalent practical experience in the human services field, or
· A combination of educational and professional experience equivalent to the above two criteria

OTHER REQUIREMENTS:

· Must pass criminal background screening
· Must possess a valid California Class C Driver’s License and be insurable
· Experience with and knowledge of computer use and office and data management software

Responsibilities

Participate in periodic training and cross-training opportunities provided by No Wrong Door Network Partners and various other local, State, and national organizations to develop and/or maintain subject matter expertise in aging and disabilities services supports, including but not limited to Enhanced Information, Assistance, and Referral (I&AR); Long Term Services and Support (LTSS) Options Counseling; Short-Term Service Coordination; and Transition Services.
Assist in the development and provision of site operational and event coordination for ADRC-related consumer outreach, and for information, education, and training events for consumers and fellow community professionals.
Assist in maintaining program records, assist in on-going analysis, and prepare and produce a variety of reports related to program services, service delivery, and related outcome and quality measures.
Listen patiently to understand the needs of callers seeking information, and ask follow-up questions as needed to determine the best options to support the health & well-being of the caller.
Assist in updating and maintaining a publicly searchable local resources database of aging and disabilities services and supports for San Benito County.
Surveying, documenting, and reporting on consumer outcomes; the effectiveness of information, referrals, or counseling received; and measurable elements of consumer satisfaction.
Maintain accounts, providing thematic messaging, and creating on-going content for social media and digital marketing related to the program.
Provide direct service, support, and coordination in the delivery of high-quality enhanced Information, Referral, and Assistance, Options Counseling, Short-Term Service Coordination, and Transition Services.
Assist in developing and delivering ADRC-related emergency preparedness information, resources listings, training, and site activation in the event of an emergency or disaster declaration.
Provide insight, understanding, and local site perspective for program design, service delivery and integration of the aging and disabilities No Wrong Door services and supports network.
Serve as a coordination and development liaison to the project’s other No Wrong Door partners.
Completion of additional duties and responsibilities as assigned.
