JOB OVERVIEW

We are seeking a passionate and organized Program Coordinator to lead and coordinate home support services for seniors aged 65+. This role ensures seniors get the support they need—from personal care to social engagement—while building community connections and improving overall well-being.

REQUIREMENTS

  • Degree or above of Social Work from an accredited School of Social Work
  • Current registration and in good standing with the Ontario College of Social Workers and Social Service Workers
  • Current registration with the Ontario Association of Social Workers .
  • At least 3 years case management experience, in a primary health care/community-based/family setting
  • Demonstrated case management and coordination experience
  • Familiar with in-home support and assisted living services
  • Experience in family counselling and Geriatrics/working with seniors an asset
  • Valid CPR/first aid certification
  • Ability to speak Cantonese and/or Mandarin is an asset
  • Possession of an Ontario Driver’s Licence is an asset
  • A valid vulnerable sector police record check during employment
    Job Types: Full-time, Fixed term contract
    Contract length: 12 months
    Pay: $25.00-$30.00 per hour
    Expected hours: 35 per week

Responsibilities
  • Coordinate and manage various programs and services for seniors and community members
  • Provide leadership to a team of home support workers including supervisory work related to staff and daily operations
  • Monitor targets and compile quarterly statistic reports
  • Conduct need and psychosocial assessments and therapeutic intervention/counselling to clients to assist them to achieve optimum social functioning and psychosocial wellbeing
  • Develop, initiate and coordinate individualized support plans for clients and provide ongoing resource management and case management as appropriate
  • Provide counselling services to individual and caregivers
  • Advocate for clients and their families in matters affecting senior’s health and wellbeing
  • Identify and support access to community, volunteer and professional services and programs for clients
  • Perform cross-functional and other responsibilities, as assigned and/or requested
