POSITION SUMMARY

Evergreen Healthcare Solutions, Inc. is looking for a Program Manager. The ideal candidate will be responsible for liaising with their contemporaries in other organizations, with the goal of securing referrals for DDA services on behalf of Evergreen Healthcare Solutions, Inc. This role requires program management experience, mentorship ability, former CCS experience a must, a strong leadership skills and experience working with individuals with developmental disabilities, specifically under the DDA waiver programs. The right candidate will have the ability to network with, and experience dealing with, members of the CCS community.
Candidates with extensive experience in CCS roles will be given priority consideration.

EDUCATION & EXPERIENCE:

  • Bachelor’s degree in social work, psychology, or a related field (master’s degree preferred).
  • At least 5 years’ experience working with individuals with developmental disabilities, 3 of which should be in the role of Program Manager
  • Strong networking skills and the ability to lead a team effectively.
  • Must have excellent relationship management skills, and the ability to think strategically and develop innovative solutions.

Responsibilities
  • Secure referrals for DDA Services by networking and liaising with members of the CCS community
  • Arrange and attend referrals meetings, negotiate the terms of service for the referral in order to secure the most favorable terms for Evergreen Healthcare Solutions, Inc.
  • Ensure that all Person-Centered Plans are accurate and implemented correctly
  • Write outcomes, goals, and strategies as part of the Person-Centered Planning process and ensure that progress is being made toward the achievement of those outcomes, goals, and strategies.
  • Ability to acquire clients and client referrers
