Program Manager Part-Time Justice Services Division Agency Overview Felton Institute responds to the needs of our community by providing innovative, evidence-informed social services that transform lives. Program Description FTC - STARR is a collaborative partnership program designed to meet one of the most critical community care needs in San Francisco – providing outpatient case management, and wraparound services for adults with co-occurring substance use disorder who have had contact with the criminal justice system. We provide intake, assessment, and triage assisting individuals who are diverted or discharged from jail to immediately access SUD treatment. FTC - STARR provide a Case Management services to work with families referred to FTC at 100% FTE. Hours worked will be reported by Contractor via monthly or quarterly invoice. The Case Manager will be experienced in providing outreach, engagement, referral, and linkage services. In collaboration with other members of the FTC team, the Stabilization Case Manager will be responsible for: serving as a member of the FTC team and Perinatal Stabilization Team (PST); outreaching to participants referred though PST or other FTC referral sources, providing assertive case management, skill-building, crisis intervention, and harm-reduction support to assigned FTC participants; working collaboratively with FTC partner agencies to provide wrap around support and to develop, with participant input, an effective Family Wellness. Care Plan; utilizing Motivational Interviewing, Cognitive Behavioral Therapy skills, and other evidence-based and evidence-informed practices suitable for diverse participants; supporting participants to access appropriate community-based resources, including accompanying participants to appointments and/or court dates; monitoring and tracking participants’ progress in achieving their goals and objectives; working collaboratively with FTC partner agencies and other community-based, social service agencies to help maintain stability, promote recovery and/or mental and health care support; participating in FTC case conferencing and status hearings, Child and Family Team meetings, and another participant- and family-focused collaborative meetings. Participate in FTC governance meetings, such as twice monthly Core Team meetings, grant management meetings, etc.; complying with requirements for documentation and reporting. adhering to SAMHSA data collection guidelines and requirements; adhering to laws regarding confidentiality and reporting requirements and maintain necessary compliance documentation. planning and facilitating skills groups in person and virtually as needed; other duties as required by the program. Job Descriptions Under the supervision of the Division Director, the Program Manager follows the principles of harm reduction, trauma-informed care, and recovery and wellness to engage clients in therapeutic interventions, link them with appropriate social services such as transitional housing, employment, public benefits, and other services to support them on their path to re-entry. The Program Manager will be highly visible in the community, along with team members. The Program Manager will attend community events and act as a liaison with community partners to support the goals of the FTC - Unified Command System, the Department of Public Health, and other social service agencies. In addition will be responsible for ensuring that quality services are delivered to community members within the guidelines of Agency and programmatic goals and mandates, actively participates as a member of Felton’s management team, completes all required trainings, and attends all required Agency meetings. The Program Manager may conduct community outreach alone or with members of the Felton team or the Unified Command System. Program Manager Responsibilities Works a flexible full-time schedule to accommodate program/client needs. Oversees, reviews, and maintains all required documentation of program activities, and assists in tracking data related to program activities, as needed Conducts aggressive community outreach and engagement in order to provide services to clients safely within the community, as needed, and within the clinic, as appropriate Maintain Quality and Impact Objectives and other appropriate client service goals after each client contact or service, in accordance with mandated standards of Felton Institute and FTC - STARR program requirements Contributes to team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas Provides information regarding FTC - STARR program to Prop - 36 participants in or being released from the SF County Jail Other duties and responsibilities may be assigned as required to meet program goals. Minimum Requirements and Qualification Master’s Degree (Public Administration, Counseling, Social work, Marriage and Family Therapy, Public Health, or related topic), LSCW, LMFT preferred Bachelor’s degree in a field with emphasis on statistical, analytical skills and information systems Minimum of three years of direct experience working with transition age youth and/or adults who are experiencing substance use disorders, mental illness, homelessness and/or related challenges Minimum of one year of experience supervising and/or managing a team Desire and ability to work from a strength-based, client-centered model with an emphasis on harm reduction practices Ability and willingness to provide non-judgmental encouragement to clients is essential Ability to work cooperatively on a multi-disciplinary team, and to work with team members and clients with various life experiences and educational backgrounds Knowledge and experience working with computers, including word processing and data entry programs Able to effectively organize data and present information in group settings Able to work effectively both independently and collaboratively Strong interpersonal and communication skills – both oral and written Outstanding organizational abilities; good problem-solving skills Verifiable clean driving record (preferred) Valid California Driver’s License, insurance, and ability to travel as required to perform duties (preferred) Additional Information: Reports to: Division Director Job Type: Temporary (4 months) Work Schedule: Onsite FLSA Status: Exempt Salary Range: $16,000 (adjusted salary for four months) Benefits (subject to election and eligibility): Fully Paid Medical and Dental Plans; Health Reimbursement Card, Paid Time Off (personal, vacation, and sick time), 403B with company match.
