Organization: SMF-Sutter Med Foundation- Central Position Overview: Serves as the primary point of contact and care navigator for professional and collegiate athletic partnerships. This role provides concierge-level coordination of care, ensuring athletes, coaches, and team staff have seamless access to Sutter’s clinical network, including physician specialists, imaging, surgical, and rehabilitation services. Functions as a program manager and clinical liaison with advanced skills in communication, logistics, and clinical coordination, bridging the gap between athletic organizations and Sutter Health’s providers. The position is responsible for managing end-to-end coordination of care, facilitating referrals, scheduling expedited appointments, and ensuring confidentiality, continuity, and quality across the continuum of care. *This position is hybrid and will require travel as needed within and outside of the Sacramento area. Job Description: EDUCATION: Equivalent experience will be accepted in lieu of the required degree or diploma. Bachelor's: Business Administration or related field TYPICAL EXPERIENCE: 8 years recent relevant experience. SKILLS AND KNOWLEDGE: Knowledge and understanding of strategic planning, including group facilitation and plan development. Knowledge and understanding of business plan development, including qualitative and quantitative research and analysis. Well-developed communication and interpersonal skills. Ability to interact with a wide variety of internal and external customers, including patients, physicians, payors. TRAVEL: Travel is required as needed for this position.
