AIDS Foundation Chicago (AFC) mobilizes communities to create equity and justice for people living with and vulnerable to HIV, chronic conditions, and housing instability. Through advocacy, direct services, and strategic partnerships, AFC works to improve health outcomes and promote housing stability across Illinois. We are seeking a Program Manager of Permanent Supportive Housing to support special initiatives within the Flexible Housing Pool (FHP), including the Reentry Program, Healthcare Transformation Collaboratives (Wellness West & SSHCO), and the Healthy Futures program. This role is ideal for someone who thrives in a dynamic environment, excels at cross‑sector collaboration, and brings experience in homelessness, reentry, or supportive housing systems. This position plays a critical role in expanding access to permanent supportive housing for individuals experiencing homelessness in Chicago and Cook County. The salary range for this position is $50,000 to $54,000 annually, commensurate with experience. ESSENTIAL FUNCTIONS & RESPONSIBILITIES PROGRAM MANAGEMENT & OVERSIGHT * Facilitate smooth transitions from outreach to housing placement for participants in special initiatives * Assign newly enrolled participants to partner agency case managers and ensure high‑quality case management support * Convene and lead monthly Systems Integration Team (SIT) meetings with case managers, healthcare partners, and IDOC * Serve as the primary liaison to partner agencies, providing problem‑solving support and technical assistance * Collaborate with the Landlord Engagement Team on move requests and lease renewals * Monitor partner agency utilization of Client Assistance Funds and manage Chicago Furniture Bank voucher access CROSS‑TEAM COLLABORATION * Partner with FHP Senior Program Managers to standardize case management practices * Ensure seamless integration between housing, healthcare, and criminal justice partners * Work closely with the Behavioral Health Team to embed mental health and wellness supports * Collaborate with CHH and AFC leadership to drive system‑wide improvements in housing services QUALITY MONITORING & IMPROVEMENT * Train and onboard new partner agency staff in FHP policies, procedures, and data systems (CaseWorthy, HMIS) * Ensure case managers are trained in Housing First, Harm Reduction, Client‑Centered, and Trauma‑Informed Care * Conduct performance monitoring, including data accuracy reviews, monthly service reporting, and annual site visits * Maintain high‑quality data collection and reporting for funders WHAT WE’RE LOOKING FOR  * Experience in homelessness or housing systems is strongly preferred * Adaptability and the ability to manage multiple priorities across several cohorts (Reentry, Healthy Collaboratives, HUD project) * Reentry experience is a preferred qualification * HUD program familiarity is helpful due to the learning curve * Strong data quality and monitoring skills (HMIS, CaseWorthy, or similar systems) * Excellent communication skills, both written and verbal * Comfort with hybrid work and occasional site visits MINIMUM QUALIFICATIONS * Bachelor’s degree in social services or related field OR three (3) years’ experience in Housing, Homelessness, Healthcare, or lived experience of homelessness * Experience in program management, case management coordination, or direct service provision in housing or supportive services PREFERRED QUALIFICATIONS * Master’s degree in Social Services, Public Health, Psychology, or related field * 3+ years’ experience in Housing, Homelessness, Healthcare, or Criminal Justice * Experience with reentry populations or criminal justice partners * Familiarity with HMIS, CaseWorthy, or other case management databases * Supervisory experience or partner‑agency management * Lived experience of homelessness * Bilingual candidates strongly encouraged to apply KNOWLEDGE, SKILLS & ABILITIES * Strong coordination, problem‑solving, and multi‑tasking skills * Understanding of Housing First, Harm Reduction, and supportive housing models * Experience with data systems, reporting, and compliance * Ability to lead meetings and facilitate cross‑sector collaboration * Strong relationship‑building skills across housing, healthcare, and government sectors * Commitment to equity, trauma‑informed care, and culturally responsive service delivery WORK ENVIRONMENT & TRAVEL * Hybrid schedule with 1–2 days in the office per week * Occasional local travel within Chicago and Cook County * Occasional off‑site meetings with partners BENEFITS * Multiple health plan options, including some at no cost * Dental and vision insurance * Retirement plan with Roth or pre‑tax contributions * Generous paid time off * Flexible scheduling (hybrid/remote/on‑site depending on role) * Employer‑paid life, AD&D, and disability insurance * Employee Assistance Program (EAP) * Additional perks: FSA, pet insurance, legal assistance, and discounts at 300,000+ vendors
