PROGRAM OFFICER – AGED CARE - DALLAS

Kogan People Solutions, recruiters, are delighted to be working with a treasured organisation which exists solely to empower migrants and refugees to feel at home in Australia. From migration and settlement support to culturally responsive aged care, their tailored services are designed to meet the unique needs of every individual and family.
They now wish to appoint a Program Officer (Aged Care) to work in their Dallas office. Their aged care speciality offers a range of services to help older people come together, stay active, live independently and engage in the community The successful applicant will be able to demonstrate deep expertise in culturally inclusive healthcare, preventative care planning, and community health engagement.

Responsibilities

THE ROLE IS FULL TIME, AND ONGOING.

Location – Dallas Vic
Salary – negotiable in the range $80k to $90k plus superannuation per annum.

DUTIES AND RESPONSIBILITIES WILL INCLUDE:

  • Collecting and analysing clinical data including vitals, mobility status, and medication adherence to inform tailored care planning.
  • Developing and implementing evidence-based health promotion programs on topics such as fall prevention, dementia awareness, continence care, and healthy ageing.
  • Creating culturally appropriate health education materials and working in collaboration with interpreters and community stakeholders.
  • Coordinating and leading group sessions, workshops, and health expos tailored for culturally and linguistically diverse (CALD) aged communities.
  • Conducting culturally sensitive biopsychosocial health assessments to identify risks, chronic illnesses, mental health concerns, and functional limitations in aged care clients.
  • Integrating wellness and prevention strategies into individual home care and residential aged care plans alongside case managers and health practitioners.
  • Maintaining accurate electronic health records using systems such as Goldcare, ma Procura and AutumnCare.
  • Preparing internal and external technical reports for audits, evaluations, and funding compliance.
  • Collaborating with multicultural communities’ faith based organisations and local networks to co-deign aged care services
  • Providing health assessments and education to clients at home or in remote places by using online communication methods
  • Ensuring clients are able to understand health information and changing how it’s explained to ensure that client groups are helped to understand that information.
  • Acting as a liaison among clients, families, GPs, allied health professionals, and specialist services to ensure coordinated care
  • Administering and interpreting diagnostic screening tools such as the Geriatric Depression Scale, Mini-Mental State Exam, and Nutritional Risk Screening.
  • Ensuring all programs align with the National Aged Care Quality Standards and state-based health promotion policies.
  • Conducting health and safety risk assessments related to infection control, medication safety, and manual handling.
  • Participating in incident investigations and service improvement reviews to enhance the quality and safety of aged care delivery.
  • Staying current with relevant aged care legislation including the Aged Care Act and the Charter of Aged Care Rights.
