Description Position Summary: The Program Specialist is responsible for fully supporting the Behavioral Health Clinical Services Manager by assisting with the outreach and in-reach of multiple Roots programs. Will work in collaboration and continuous partnership with Program Coordinators, to provide community outreach in order to link individuals to the Roots Community Health Center for integrated primary care, behavioral health, enabling resources and other supportive services for which they are eligible. This position requires flexibility in hours that may include working evenings and weekends. Duties and Responsibilities: Conduct community outreach and in-reach to connect targeted populations with Roots programs, including evenings/weekends. Provide intake/assessment, evaluate eligibility, set up appointments, and complete required documentation. Support Behavioral Health staff to ensure program deliverables align with scope, budget, and timelines. Conduct bi-weekly meetings with clinicians to provide individualized updates of clients and ensure continuity of care. Act as a liaison to the community and partner agencies, maintaining culturally competent communication. Make outbound/receive inbound calls, provide detailed program information, and generate leads through referrals, canvassing, and research. Maintain accurate and timely reports, including outreach metrics and client follow-ups. Provide support in implementation and coordination of activities with internal and external service agencies. Uphold confidentiality and ethical standards, travel throughout service areas, and adapt to flexible schedules. Requirements Competencies: Bachelor's Degree is required in Development Practice or Social Work Culturally competent and able to work with people from diverse backgrounds. Strong attention to detail. Excellent verbal, written, and interpersonal skills. Strong problem-solving skills. Ability to multi-task and juggle multiple priorities. Ability to use appropriate organizational skills in setting priorities for work, and a commitment to meeting deadlines. Ability to communicate well at all levels of the organization including working with organization leadership and representatives of partner organizations. High level of ethics and personal integrity; must be able to work with confidential information. Willingness to work a flexible schedule. Consistent attendance track record and prompt arrival to work and meetings. Ability to travel and navigate throughout Roots' service areas and neighborhoods. Frequent walking, standing, stair climbing, bending, stooping, lifting (25 lbs. max). Ability to operate standard office machines and equipment including keyboards. Must be able to visually observe staff to understand and interpret non-verbal behavior.
