Service Management Group (SMG), Inc. is a spinoff of Accra. We are a non-profit dedicated to improving lives through innovative home-based services and solutions. The Program Support Coordinator will support our Home Health program.
The Program Support Coordinator will interact with clients through phone calls, video calls, text messages, and emails to ensure visits and appointments are scheduled and the client’s expectations are being met with a high level of customer satisfaction. The Program Support Coordinator will collaborate and coordinate with care managers, nurse consultants, clients, caregivers, providers, county care managers, technology/IT specialists, and other stake holders. The Program Support Coordinator is responsible to monitor and coordinate all incoming program referrals. Performs administrative duties to maintain and provide support for optimal home care program operations.

Requirements:

  • Post secondary education required (e.g. associate degree, technical certification, or equivalent coursework).
  • Minimum 2-year experience in customer service or administration in an office setting.
  • Previous experience in home care or community-based services is preferred.
  • 1+ year experience working with vulnerable populations (medically complex, behavioral health diagnosis, underserved)
  • Excellent communication and interpersonal skills, with the ability to build relationships.
  • Intermediate computer skills including internal office and organizational software, and the ability to gain proficiency with role specific systems.
  • Ability to maintain a high level of confidentiality and demonstrate an objective mindset with all verbal and written information that is shared.
  • Aptitude to think strategically and creatively, with a strong focus on delivering results.
  • Ability to work with complex issues independently and exercise excellent judgement to problem solve client related and other issues in an accurate and timely manner.
  • Ability to work within a team collaborating effectively with others.
  • Ability to strengthen agency procedures and daily operations.
