JOB REQUIREMENTS

  • Diploma or Degree in healthcare-related industry or other relevant field of study.
  • At least 2 years working experience in the community care sector.
  • Basic proficiency in Microsoft Word, Excel and Power point, and other computer skills.
  • Strong planning, coordination and secretariat skills, including planning of meeting agenda, minutes-writing and report-writing.
  • Able to work well independently and as a team-player.
  • Good interpersonal, communication and networking skills, to forge partnership with community partners.
  • Good time-management skills, well-organised, able to multi-task and adapt to working demands.
  • Monday to Friday (Office hours).
  • Able to work on weekends when need arises.
Responsibilities

ROLES AND RESPONSIBILITIES

  • The Programme Lead supports Ren Ci Hospital and the partner organisations in the sub-region to build strong partnerships, align goals, establish roles and practices, collaborating and working as a team to achieve the ICCP objectives.
  • Secretariat and Coordination Function


    • -
    • Work with leadership team of the sub-regional ICCP organisations to align goals, establish roles, and develop strategies and workplan for the sub-region to achieve the deliverables specified for ICCP.
    • Work with centre managers and ground teams to implement the ICCP workplan, monitor and report on the progress of the workplan. This includes building understanding across centres/organisations and working through implementation and other challenges.
    • Work with ICCP organisations to develop and trial joint referral workflows, care coordination deliverables and other outcomes arising from the programme.
    • Engage and build networks and partnerships with other community care providers, regional health systems, grassroot organisations and other stakeholders.
    • Plan and coordinate ICCP meetings, agenda and prepare meeting minutes.
    • Administrative Function


      • -
      • Collect and consolidate the information required for the ICCP quarterly progress reports and whenever required.
      • Ensure that the endorsed reports and claim forms are submitted to AIC in a timely manner.
      • Represent the ICCP to attend meetings convened by AIC as required, and report back to the leadership team of the partner organisations.
