An exciting opportunity has arisen for a pro-active and experienced Project Manager to assert and develop palliative and end of life care processes within Dundee Health and Social Care Partnership (DHSCP) for patients and families in Perth & Kinross (P&K).
The post holder will be primarily responsible for providing project management skills in assessing need for palliative and end of life care within P&K and, in collaboration with clinicians and social care, develop pathways across all health and social care locations that relate to positive outcomes for patients and their families. You will have experience of managing project and influencing service delivery alongside contributing to service development.
You will hold a first level degree and have a qualification in project management. You will be an advocate for adopting an enabling approach and will be well versed in the principles of change management.
The ability to work autonomously and travel independently is essential. Excellent communication and team working skills are required as the post-holder will need to liaise with patients and their families and members of the health and social care team, including the public.

You will be supported to develop skills and competencies which will allow you to operate as an autonomous practitioner with limited supervision of your clinical work.

Hours of work are: 37 hours per week
Fixed Term/ secondment for 18 months
