The ideal candidate will serve as the liaison between the physician practice and various internal and external stakeholders to help nephrologists achieve value-based targets and objectives. This role will serve as the key point of contact to streamline communication with physician practices and facilitate the achievement of program goals including the improvement of clinical outcomes. The PEM will collaborate with cross functional teams including business development, clinical, operations, and business leaders across DaVita. Details: This role will support the IKC of Virginia Market, which partners with nephrologists to care for patients in Virginia, West Virginia and North Carolina. Requires regional travel as needed, in Virginia, West Virginia and North Carolina Expected travel is between 10% and 15% Essential Functions/Responsibilities: Partner and manage successful relationship(s) with Nephrologist(s) to build trust and physician satisfaction. Serve as a trusted advisor and main point of contact for physician practice. Build formal relationships with key stakeholders within the local integrated network including but not limited to: PCPs, Specialists, Dialysis Centers, SNFs, LTACs, high-volume hospital systems and transplant centers. Co-lead market ops meetings with the market director and practice physicians. Additional attendees may include: DV clinical leaders, IKC clinical leaders, market analyst, etc. Educates practices on all program requirements including participation and responsibilities for quality assurance (QA) and quality improvement (QI). Provides streamlined nephrologist engagement across DaVita IKC and other GIKC value-based care programs. Review quality metrics, clinical reports, program playbooks, program updates, and other pertinent documents with physicians. Ensure that physicians have solid understanding of program requirements and expectations. Implement meaningful process changes by assisting in physician practice assessment as it pertains to the CKCC program, leading the implementation of efficient and effective processes, and identifying support resources that result in optimal clinical/patient outcomes. Provides analysis of clinical and operational data from multiple sources and determines current trends to develop QA and QI actions appropriate for value-based program needs. Facilitates behavior change through practice QI team by Implementing ongoing meeting schedule for physician champion and practice QI team, involving other practice physicians, and practice staff as appropriate. Provides coaching, feedback, and assistance to practice for change leadership and management. Uses proven techniques of practice facilitation to influence and promote high-value healthcare. Identifies practice educational needs and matches them with available DaVita resources. Reviews and synthesizes data reports provided by NCA or DaVita analyst. Checks for data accuracy, provides recommendations to improve practice experience, and understands critical focus areas. Collaborates with DaVita and IKC clinical leadership on key findings. Other duties as assigned Education and Qualifications: Bachelor’s Degree- Preferred Proven track record of building successful physician relationships- Required At least 2 years of project management experience Prior experience in healthcare services and/or working with payers, value-based care models, and/or integrated healthcare models Process improvement training (Lean Six Sigma, PDSA)- Preferred At least five years of experience in a clinical or clinical support role- Required Advanced proficiency in Microsoft Office Products (Excel, PPT, Word) What We’ll Provide: More than just pay, our DaVita Rewards package connects teammates to what matters most.
