POSITION SUMMARY

General Summary
The Psychiatric Rehabilitation Program (PRP) Rehabilitation Specialist fulfills the roles defined by COMAR as PRP Specialist in helping to run and oversee the PRP program under the leadership of the Director. This individual also will provide direct care including assessment and treatment services to individuals and groups, as well as facilitate partnership strategies with the community. The PRP Specialist will manage direct care and ensures staff are focusing on promoting independence, recovery, and wellness by supporting participants in developing life skills, improving social functioning, and accessing community resources. The PRP Specialist works with staff to ensure Individual Rehab Plans are comprehensive, individualized, client guided and achievable. Overall, this individual takes ownership to support the program’s mission for healing, innovation, and an outcome-measured outpatient care.

QUALIFICATIONS

  • Master’s degree Psychology, Social Work, Human Services, or a related field and a Licensed Mental Health professional. or certified as a CPRP practitioner.
  • Minimum of 2–3 years of experience working in direct care with individuals with mental health challenges.
  • Knowledge of psychiatric rehabilitation principles and community resources.
  • Strong communication, organizational, and documentation skills.
  • Ability to work independently and manage time effectively.
  • Valid driver’s license and reliable transportation (for community-based work).

PREFERRED SKILLS

  • Familiarity with Medicaid/PRP program regulations.
  • Experience supervising direct care staff.
  • Experience facilitating groups and teaching life skills.
  • Bilingual abilities a plus.

PHYSICAL REQUIREMENTS

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to access and navigate each department at the organization’s facilities.
Responsibilities
  • Work with staff to provide direct skill-building and support services in areas such as daily living, social skills, vocational readiness, and community integration.
  • Work with staff to develop, implement, and monitor individualized rehabilitation plans in alignment with treatment goals.
  • Ensure regular one-on-one and group sessions with participants in home, community, and office settings are occurring, are individualized and in alignment with the participant’s IRP.
  • Ensure participants are accessing resources for housing, employment, education, healthcare, and social services.
  • Ensure staff are documenting all client interactions and progress notes accurately and in compliance with state and agency regulations.
  • Collaborate with therapists, case managers, and other providers to ensure continuity of care.
  • Ensure services promote recovery, resilience, and self-advocacy through strengths-based and trauma-informed approaches.
  • Provide crisis intervention and support when needed.
  • Coordinate team meetings, trainings, and additional supervision as required.
  • Engage in community education, outreach and referral generation.
  • Ensure programmatic goals for service delivery, regulatory compliance, quality of care and quantity of services
  • Identify areas of programmatic need and contribute to system development for program growth.
  • Develop knowledge of community resources and develop integrated relationships with outside providers and supports.
  • Ensure continuity of care between internal and external providers.
  • Ensure services are provided with client-centered and family-centered healing approach and are delivered through goal specific interventions. Includes:
  • Conduct initial interviews pursuant to history and provide a written assessment.
  • Deliver in-home or in-office services to clients assessed.
  • Regularly assess treatment progress with the client and/or the clinical treatment team.
  • Provide on-site support and feedback as needed as well as telephone contact with the client and team.
  • Provide the appropriate documentation for service delivery including treatment goals, progress notes and billing and submit said documentation as specified by Medicaid and CARF standards.
  • Ensure compliance with governing agency and reporting requirements
  • Manage and develop staff for highest level of competency and growth in alignment with WRPAROUND MARYLAND values and goals.
  • Conduct regular ongoing chart reviews and demonstrate tracking of outcomes and follow up of identified areas for improvement.
  • Maintain the clinical records and compile documentation required by WRPAROUND MARYLAND and governing agencies.
  • Participate in staff meetings, program clinical team meetings, clinical supervision sessions and in-service as required.
  • Understand and support the WRPAROUND MARYLAND philosophy and treatment modalities.
  • Ensure confidentiality regarding client issues, background and treatment
  • Exhibit clear, professional, respectful and effective communication when interfacing with families and external contacts
  • Demonstrate concern and empathy towards others as needed, while maintaining appropriate boundaries.
  • Skill in working independently and implementing time management strategies
  • Exhibit a high degree of ethical conduct, integrity and confidentiality.
  • Ensure client information is protected pursuant to federal regulatory standards (HIPAA and 42 C.F.R. Part 2)
  • Stay current with updated research and best practices.
