Company Description We are currently hiring Personal Support Workers (PSW) in Mississauga and Etobicoke to be part of a dedicated and specialized team that will deliver a highly integrated, one team, community palliative care model for the Mississauga community. Job Description As part of a tight-knit collaborative team, you will assist palliative patients with personal care and activities of daily living. You will support patients, families and caregivers –enhancing quality of life, comfort, dignity and security in patients’ end of life journey. All direct patient care activities will be performed under the direction of a Neighbourhood Home Care Specialist who is a Registered Nurse or Registered Practical Nurse. Working alongside the interdisciplinary palliative care team, the PSW provides specialized care as specified in the care plan, according to protocols, within scope of practice and in accordance with Spectrum Health Care’s policies and procedures. Provides companionship and comfort, creating a calm and respectful environment tailored to the client's needs, preferences, and goals of care. Assists person(s) with activities of daily living such as bathing, toileting, hygiene, skin care, light meal preparation, feeding and transfers. Performs light housekeeping duties such as washing dishes, taking out the garbage and laundry. Sweeping and vacuuming may be required, if indicated. Listens, observes and communicates any concerns/changes in the patient’s physical and/or emotional condition to the Neighbourhood Home Care Specialist. Travels within assigned region between patients’ residences or facilities. Qualifications Have one of the following qualifications (required): Valid Personal Support Worker Certificate from a Ministry of Training, Colleges, and Universities or the Ministry of Education approved program RN/RPN Student with minimum successful completion of year-one of the program Internationally licensed Nurse, Midwife or Physician Experience providing services in a community health care setting Must have experience working with palliative care patients Successful completion of one of the following an asset: Fundamentals of Hospice Palliative Care; or Core Concepts of Fundamentals for PSWs; and Additional and on-going relevant palliative education Current CPR and First Aid Certificate Current Police check suitable for working with the vulnerable sector. Excellent English communication skills, written and verbal Ability to interact with others in a caring, tactful and courteous manner Ability to work independently within the scope of the role and prioritize workload.
