Description The health services that became the Anthony L Jordan Health Corporation (Jordan Health) began more than 100 years ago, in 1904. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of Rochester, NY and in Canandaigua, N.Y. Jordan Health is now seeking a Psychiatric NP/PA who, under the guidance of the Director of Social Emotional and Mental Health/Practice Chief, is primarily responsible for providing behavioral health care to patients and family in collaboration with primary care physicians, physician assistants and nurse practitioner colleagues throughout the Jordan Health Care setting. Provides comprehensive behavioral health evaluations, consultations with providers, assists with coordination of care either within the Jordan Health Care setting or referrals to community mental health centers. Performs in short term, episodic model, under the regulations of the Department of Health. Requirements The Psychiatric NP/PA will ensure to: · Obtain psychosocial and behavioral health history on patients upon family medicine referral, throughout lifespan. · Complete physical and mental health assessment according to need and established clinical guidelines. · Provide counseling specific to developmental needs including symptom management, medication compliance, substance use, risk reduction, family planning, school/work issues and family/relationship issues. · Manage common acute behavioral health illnesses, chronic behavioral illness and health maintenance needs, using of clinical laboratory and diagnostic tests/imaging and procedures. · Help to advocate for patients' needs within the health care system and with community agencies. · Refer to specialty services as appropriate, including makes community mental health referrals when long term care is needed. · Assist Family Medicine and Behavioral Health staff in planning future activities of the Department, implementing and assessing Department activities. · Assist in orientation and precepts graduate nursing students, residents, medical students and new staff as needed, when applicable. · Comply with NYS guidelines regarding licensing, prescriptions, and record keeping. · Complete duties as assigned. Education and Experience Required: · Masters Degree in Family Psychiatric Nurse Practitioner, Physician Assistant in Psychiatric Mental Health. · One year of clinical experience working within the field of substance abuse and/or behavioral health. · Preferred candidates will have experience working with chronic mental illness. · Experience working with multicultural backgrounds. Licenses and Certifications: · NYS RN, NPP, PA licensure; National Certification. · BLS through the American Heart Association · NYS Infection Control Certification Special Skills, Knowledge Required: · Bilingual preferred.
