Scope of Position: The ACT Psychiatric Nurse Practitioner at The Bridge is a full-time, field-based clinician and integral member of an interdisciplinary Assertive Community Treatment team serving adults with serious behavioral health diagnoses, substance use issues, and co-occurring medical conditions. This role provides comprehensive psychiatric care to individuals with histories of multiple hospitalizations who have not been successful in traditional office-based settings. Working from a Harm Reduction and Trauma-Informed perspective, the Psychiatric Nurse Practitioner delivers person-centered services primarily in the community (approximately 80% field-based), while recognizing and addressing the impact of systemic racism on the populations served. Essential Position Functions: Conduct psychiatric assessments and evaluations. Prescribe, monitor, and administer psychotropic medications. Provide crisis intervention services to clients during regular business hours and consultation during off-hours. Participate in clinical case conferences, team meetings, High Risk conferences, quality improvement, evidence-based practice implementation, and policy/procedure development. Assess and monitor the physical health of clients. Complete all clinical documentation as required by payers, funders, and regulatory authorities via an Electronic Health Record. Complete all required administrative paperwork. Qualifications Board-certified Psychiatric Nurse Practitioner, with appropriate experience or a strong desire to work in this setting; new graduates welcome. Bilingual Spanish proficiency is highly desirable. Programming hours are Monday through Friday, primarily from 9:00 AM to 5:00 PM. Competitive salary and benefits package.
