Description BMS Family Health and Wellness Centers is seeking a dedicated and compassionate Psychiatric Nurse Practitioner (PMHNP) to join our integrated care team. In this role, you will provide comprehensive mental health services across the lifespan, supporting both pediatric and adult populations in a collaborative, patient-centered environment. The Psychiatric Nurse Practitioner will play a key role in delivering high-quality care, including psychiatric evaluation, diagnosis, treatment, and ongoing management of acute and chronic mental health conditions. This position offers the opportunity to work within a multidisciplinary team committed to improving access to behavioral health services in a primary care setting. ________________________________________ Key Responsibilities Conduct comprehensive psychiatric evaluations, risk assessments, and diagnostic formulations Develop and implement individualized treatment plans, including medication management and therapeutic interventions Provide ongoing care for patients with acute and chronic mental health conditions Collaborate closely with physicians, social workers, nurses, and other care team members to ensure coordinated, holistic care Participate in care coordination with internal and external providers as needed Engage in preventive care activities, including mental health screenings and early intervention strategies Maintain accurate, timely, and compliant documentation in the Electronic Health Record (EHR) Participate in regular chart reviews and quality improvement initiatives Contribute to monthly reporting, including productivity and clinical outcome measures Support and adhere to organizational policies, procedures, and clinical protocols Participate in the on-call schedule as required Collaborate with leadership, including the Assistant Medical Director and Nurse Manager, to support clinical excellence and regulatory compliance Ensure compliance with Managed Care, SBHC contracts, and Federal and New York State regulations ________________________________________ Requirements Qualifications Active and unrestricted New York State license as a Nurse Practitioner Board Certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP) Minimum of two (2) years of clinical experience as a Nurse Practitioner, preferably in behavioral health or integrated care settings Strong clinical assessment, diagnostic, and treatment planning skills Excellent communication and collaboration skills within interdisciplinary teams Commitment to delivering compassionate, culturally competent, and patient-centered care Experience with Electronic Health Records and clinical documentation standards Why Join BMS Family Health and Wellness Centers?
